Abstract

Advance care planning, which advocates for patient-centeredness, stretches beyond plan documentation. Instead, it incorporates value systems of patients and ensures that these values are aligned to the patients’ healthcare goals. The process entails trained facilitators’ decision to lead the patients through various questions to determine and predict their beliefs and preferences, as well as personal goals for care; especially if the patients, after some time, are unable to speak for themselves or approach the end-of-life. In the ICU unit, there is a need for quality improvement in this area because the process enables the medical power of attorney and the patients to understand preferred decisions in different situations
